I understand and agree that KCMA exclusively uses Electronic Fund Transfer for my student's monthly tuition. Cash, checks, credit cards or debit cards are not accepted for tuition. I agree to have sufficient funds available in the provided bank account to cover my student's monthly tuition. I understand that a reasonable late fee may be assessed if my bank declines an EFT transaction.

I understand and agree that a 24 hour notice is necessary to receive a make-up lesson and that make-up lessons are limited to 1 per quarter, 4 per year. Make-up lessons do not accumulate and must be used quarterly or they are forfeited. I understand that a make-up lesson may be scheduled with an instructor other than my student's primary instructor. I understand that notice is best given by calling or texting 816.505.5005 even on Sundays or by sending an email to lessons@kcmusicacademy.com.

By signing this agreement, I acknowledge that I have read it, understand it and agree to comply with all of its terms. I further understand that by signing this agreement that I am entitled to receive a carbon copy, signed and dated by both parties. I understand that if I ever feel that my rights are being violated in any way that I may contact Missouri Consumer Affairs at 800.392.4211.
